Please submit application proposal including type, objectives, content and target participants of activity and event period (with set up and dismantle dates). Venue layout plan and implementation plan should be included for application of Public Open Space.

DECLARATION AND CONSENT OF THE ORGANISATION:
- I/We hereby declare that all the information given in this application is true and accurate.
I/We understand that any inaccurate information will make the application invalid. - I/We hereby consent that the information provided in this application form will be used by the Urban Renewal Authority to process the application and conduct evaluative studies and sharing sessions. All supporting documents provided by me/us in relation to this application will not be returned to me/us irrespective of whether this proposal is successful or not.
- I/We have read and understood the Booking Application and User Guidelines. I/We agree to observe the provisions contained in the aforesaid document upon successful application.
- I/We agree that should this application be successfully accepted, I/we shall pay the licence fee and deposit in accordance with the invoice amount issued by the email of 7 Mallory Steet application.
